Unless you are a Power of Attorney (POA) or possess Legal Guardianship rights over the individual that you are seeking an update on, or the applicant has consented to us disclosing information to you on the Release of Information form, the DFA is unable to confirm or deny any information. If you have the Power of Attorney or Legal Guardianship for an applicant, please provide this to the DFA along with your ID. If the person you represent is of unsound mind and unable to make decisions for themselves, a POA is not valid in this instance, and legal guardianship is required.
