“What is a representative?”
- A representative is a person who you have authorized to access information regarding your case, and act on your behalf. You can revoke this person’s authorization at any time.
“How do I assign a representative?”
- First you’ll need to fill out our Authorization of Representative Form [ATTACHMENT LINK]
- Alongside the form, you will need to provide identification for the representative and one of the following documents:
- A letter you’ve written and signed
- A medical document
- A legal document
Your request will then be reviewed by our Department and accepted or denied.
“Can I remove a representative?”
- Of course. A Representative can be revoked at any time by filling out the Revoke Authorization of Representative Form [LINK HERE].
The Department may also revoke the representative status of a person if it is suspected that the authorization was not freely given, or if the assistance is not being used for your benefit.
